MAIN BODY

Description

In this reflection, I will emphasis on presenting insights related to global and local shortage of Healthcare Workers [HCWs], specifically nurses. Furthermore, I will assess how shortage impacts quality and management of care for cancer patients in a non-profit organisation within Hong Kong. HCWs shortage refers to the situation when healthcare settings suffer from workforce crisis due to lack number of HCWs in against of patients. In this situation patients exceeds while HCWs remains less; this disrupts the management of care procedure. The workforce shortage has been recognised through long shifts, stressful working environment, decreased investment in training and inconsistent support to workers (Ross, 2022). All of these domains adversely impacts healthcare services significantly in specialised fields such as- End-of-life care and palliative care. Nursing shortage negatively impacts quality of services for cancer patients. Individuals suffering from cancer require palliative care which means they needs consistent supports; nurses are accountable for providing holistic support to patients which comprehends with physical, emotional, mental and spiritual support (Naseem, Naseem and Amir, 2024). However, shortage of HCWs leads to create burden on existing nurses that often results in compromising with patient care and safety. When nurses are overburdened with work then they are not able to perform their practices effectively and this drastically influences patient safety.

Feelings

As a nursing student when I learned about HCWs shortage then this led to evoke the feelings of frustration; I felt anxious as this shortage significantly impact patient safety. In my learning I got to know that patient safety is an important aspect and nurses are accountable for providing consistent care to patients. Vulnerable cancer patients require end-of-life services and thereby, during this learning I had an urgency to understand how nurses adapt such situation despite facing multiple challenges. I have recognised the importance of advocacy and resilience within own profession. I was emotionally disrupted when I got to know how much it is difficult for nurses to compromise with such situations. They are overburdened with work as they have to look upon patients consistently and this pressure often hampers their productivity and drastically impacts their care practices.

I realised that patient safety is at the forefront of healthcare services and therefore, it is important for healthcare setting to implement a robust mechanism that assures support to HCWs so their retention rate can be increased and they would be able to support patients. Support to nurses must be ensured as this complies with enhancing their practices; ineffective management at healthcare settings and stressful environment contributes in shortage of nurses and therefore, significant focus should be implied on the root cause.

Evaluation

From the evaluation of presentation I have analysed that HCWs shortage negatively disrupts multidisciplinary approach specifically in cancer care. For example- this leads to increase workload for existing nurses that significantly reduces time for undertaking patient-centred care practices and communication. Nurse cannot invest appropriate time in patient care and this leads to potential errors which further threatens patient safety in drastic manner. Hence, this situation has been highlighting about the significance of team management, collaboration and innovative care models. The focus requires to be implemented on undertaking inter-professional collaboration, task shifting, implementing appropriate leadership approaches and allocating equal roles and responsibilities. These practices support in mitigating work pressure and contribute in managing workforce shortage.

Montgomery et al, (2021) elucidated that adequate staffing levels assure timely delivery of care services to patient and promotes patient-centred care. This leads to enhance patient satisfaction to greater extent. However, Cho and Steege (2021) depicted that it is important to develop an inclusive environment at the settings where nurses can feel motivated; appropriate leadership approaches such as transformational style enables leaders to develop such environment in which nurses are consistently motivated and encouraged. This leads enhance sense of belongingness among nurses and inspires them to work in cancer settings and this reduces issues related to workforce shortages. Therefore, it is important to imply focus on this area so that complexities can be managed.

Analysis

From the analysis this is evident that stressful work environment, limited opportunities, burnout and lack of support are the major causes leading to shortage of staff. These factors are significantly contributing in creating major complexities and diminishing the ability of nurses for fulfilling their roles specifically in complex settings. In palliative care it is important for nurses to adopt patient-centred approaches which are pivotal for providing holistic support to patient. Benner's Novice to Expert theory states that experience and education are foremost components that contribute in enhancing nursing skills (Lu et al, 2022). However, shortage hinders this progression and further results in compromising with care quality.

Nurses require consistent education and thereby, it is important to organize training programmes by management team of cancer in NGO settings. However, when there is less number of workers then there are no educational programmes organized and this negatively impacts working abilities of nurses (Alsabri et al, 2022). Inadequate staff arrangements links with increase in medical errors, nurse burnout and reduces patient satisfaction. Through own presentation I have analysed negative impacts of workforce shortage specifically on patient health outcomes and I realised that it is important to take significant actions in this area. Nurse leader should adopt transformational approach and subsequently, task should be allocated among nurses. Interaction with nurses should be developed and their issues must be analysed. In this manner, support to nurses can be assured and this increases their retention within Hong Kong healthcare settings.

Conclusion

This reflection led to develop significant insights related to issues aligns with HCWs shortages; it is important to address these complexities as this links with patient safety. Higher workload on nurses reduces their motivation to work in productive way and increases potential errors. This creates drastic impacts on patient safety and therefore, it is pivotal to adopt significant strategies such as- investing in training, improving work environment and fostering interprofessional collaboration. This reflection has made me realise that nurses in palliative care settings requires consistent support; nurse responsibilities are merely not limited to looking patients. They also seek of career opportunities so they can learn about effective ways and accordingly, they provide holistic support to patients. Therefore, nurses should be advocate regarding the policies that ensures continuous professional development, promotes inclusive environment and improves resilience capabilities for adopting change, particularly in sensitive areas like End-of-life and palliative care services. I will use this learning in future context and in the role of nurse my core motive would be supporting patient while providing holistic support to them. Additionally, I will communicate HCWs shortage related concerns with seniors so they can become aware about the issues experienced by nurses and how it impacts patient safety.

Action plan

Moving ahead, I will focus on improving own resilience and leadership skills through consistent learning. I will participate in leadership programmes and training and within eight months I would be able to develop these abilities. Through this reflection I got to know that nurses need to have effective resilience capabilities so they can tackle drastic situations I want to improve my resilience abilities so I can effectively adopt such situation and ensure my support to patients. In order to improve own resilience abilities I will practice mindfulness and this supports in enhancing own emotional intelligence abilities (Al-Mugheed et al, 2022). I will develop this skill in six months and take feedback from my seniors. It is important to support nurses, especially when they are suffering from stress and work pressure. Therefore, I will focus on implementing effective leadership approaches and accordingly, needs of nurses would be analysed. This supports me in taking appropriate actions while encouraging nurses. Moreover, I will engage in interprofessional teamwork training and grab opportunities related to enhancing own palliative care practices specifically during nurses shortage. My motive is to enhance nursing practices and promoting patient-centred practices in non-profit healthcare services.

CONCLUSION

Conclusively; I can say that this reflection led to develop significant insights regarding patient safety, HCWs shortage and its implication on healthcare services. Workforce shortage in healthcare settings can lead to develop drastic impacts on patient health outcomes. Thus, it is important for nurse leader to create an inclusive environment within healthcare settings and accordingly, all nurses should be supported. Consistent interaction with nurses must be implemented as this supports in analysing their issues. Furthermore, I got to know that organising training is pivotal for providing wider professional development opportunities to nurses and this also improves their potential and reduces burnout with palliative care settings.
