The above article is based on depicting the long-term outcomes of person-centred care for adults suffering from chronic illness, a topic often explored in academic resources such as Online Assignment Help in UK. The present study was conducted through secondary research in which various databases such as CINAHL, PsycINFO, MEDLINE, Cochrane Review, Evidence-Based Medicine, and ProQuest Dissertations were analysed. All information was collected from articles published between 2017 and 2022. The study identified that person-centred care is multidimensional and helps in providing numerous services to patients (Cano et al., 2023). This medical model also supports overall well-being and health as it facilitates regular and more positive interactions among health professionals. Additionally, it helps reduce psychological stress among staff and enhances the caregiving skills of family members. However, the current study lacks information regarding the perceptions and views of other stakeholders, which should be explored in future research for a more comprehensive understanding of the impact.

The current article examines the impact of implementing person centred care model within rehabilitation hospital for children. For gaining in-depth information related to the research topic, a semi structured interview was initiated on 12 individuals that include nine healthcare professional and 3 patients or their parents. It has been identified that after gaining training related to person centred care professionals were able to create a bond with patients that help in better understanding their issue. Further, WE CARE road map support professionals in effectively implementing person centred care in the healthcare setting. Further, professionals describes that person centred care helps in working as the partner with the patients which allows them to freely share their point and support in providing better treatment to patients (Lewandowski et al, 2021). Further, patients and their family members also feel improvement in the quality of services in the person centred care model. Further, the study depicts that PPC could be successfully implemented by regularly monitoring and providing feedbacks to professional.

The above mentioned article is related to significance of education intervention for nurses as to provide better person centred care in long term care setting for older people. For initiating the current study, five databases from CINAHL, Eric, Cochrane and PsysINFO were analysed. In this article, 2587 citations were included as to provide accuracy of the information. For conducting study five themes were formed which includes interaction and caring culture, job satisfaction, Person centred intervention, older people quality of life and nursing activities. It has been identified that continuing education support in reducing agitation and neuropsychiatric system and support in promoting positive relationship among old people and nurses (Pakkonen et al, 2021). Further, nurses developed knowledge regarding measures to meet people’s need and support in providing quality care to older people. However, the study ignores the perspective of older patients in the study which creates issue in gaining wider understanding of research issue.

1.2 Process of identify relevant sources

For identifying relevant sources, five step process was followed which includes choosing topic, initial searching, identifying key source, studying source and selecting article (Dergaa et al, 2023). Under this, I was involved towards identifying the topic for the annotated bibliography. After this, I have focused on conducting initial search for collecting the entire available article. Then I was involved in critically evaluating the entire collected article as to identify the time and situation of conducting study which help in selecting most recent article and sources. Referring overall assessment, it can be stated that CRAAP model has been considered for evaluating the credibility and reliability of the sources undertaken (Flanagin, Winter and Metzger, 2020).

1.3 Assess creditability of sources

For ensuring creditability of all the sources, I have critically evaluated the time of conducting study. All the selected articles were belonging to past five years which indicates high reliability and accuracy of data (Chang, Zhang and Gwizdka, 2021). Further, I have evaluated the attached reference list in study as to identify the actual source of data which helps in accessing overall creditability of the articles.
