MAIN BODY
Need assessment
The needs assessment is defined as assessing and identifying the specific needs of the population who are facing the issue of chronic illness. For the effective solution of the problem, it is necessary that the effective type of need of the individual and the patient is assessed so that better working can be achieved (Kalantar-Zadeh, Li and Tantisattamo, 2021). The different types of need assessment are as follows-
- Physical need- it includes the fact that when the person is suffering from this disease then there are different needs in relation to the physical aspect, such as the uptake of medicines, dietary changes, indulging in physical activity, regular check-ups and many other types of medication adherence.
- Emotional need- further, the need assessment also addresses the emotional needs of the patient and the family. This includes the assistance provided to the patient about dealing with the diagnosis problem so that the treatment can be taken. Also, it includes providing mental support to the patient so that they are in a position to cope with the treatment.
- Social need- the need assessment also emphasises understanding the social needs of the patient and family as well. This is needed for the reason that social support assists the person in recovering from the disease and trying to get support from the caregiver as well (Kalantar-Zadeh et al, 2021). Also, the social need includes providing financial assistance so that better services are provided to the patient as well.
Self-management and empowerment
For better working and treatment of the patient, it is necessary that self-management and empowerment need to be done so that the patient can try to recover from the issues being faced by they are facing. Thus, patient self-management needs to be present so that the work and treatment can be improved. Patient self-management is defined as the various skills and techniques which need to be used by the patient so that they are able to manage their health condition and can recover from the disease (Guo et al, 2023). For the patient suffering from chronic kidney disease, it is necessary that better self-management principles are followed. - The first strategy is to offer education to the patient for self-management so that they can manage their work (What is Patient Self-Management? How to Empower Patients in Your Practice, 2025). When the patient has ample knowledge then it will help them in managing the work, and as a result patient will be in a position to improve their work better.
- Another strategy is to provide equitable access to the health data to the patient as well. This is necessary for the reason that when the patient has access to the data then they can review their progress and check their medication dosage, and all the other related aspects.
- Along with this, another strategy for empowerment is connecting with the patient and their families and peers so that they can build a better network for effective working. Thus, it will be assisting and empowering the patient in improving their condition.
Community resources
The community resources are those that are used by the community to meet the needs of the patient and the health needs of the whole community. This is needed for the reason that when the effective community resources are used and implemented, then the health needs of the whole society and community can be met easily. Also, for the effective usage of the community resources, it is necessary that the resources are equally accessible and effective to be used in all diseases and other related aspects (Kalantar-Zadeh et al, 2021). There are different types of the NGO’s and local organisation which assist in managing the issue of chronic health diseases. Within Hong Kong, there are different NGO’s like Eastern Renal Support Association, Renal Companion Association, KWH Renal patients Support Group and many others (Community Resources / Other Resources, 2024). Along with this, the patient having end stage of kidney failure are also provided with financial assistance and help as well to get the better treatment. Similarly, another resource includes the Community Chest of Hong Kong where the patient can take help and as a result of this the overall support can be provided to the patient (Services and Projects, 2024). All these resources are equally accessible to all the people irrespective of the category, gender, religion or any other kind of the discriminatory factor.
Within Hong Kong, there are many different types of hospitals, care homes and many other types of facilities which can assist people the chronic illnesses related to the kidneys. The hospital provides different services, like renal replacement therapy, which is available through the renal units of the Hospital Authorities. This includes providing the services that can include Peritoneal Dialysis, along with the Continuous Ambulatory Peritoneal Dialysis (CAPD), and many other different types of resources and services that are offered to the patient suffering from the issue of chronic kidney disease (Carter et al, 2022). Thus, for this, it is necessary that the resources are effectively and optimally utilised so that the overall working can be improved and better healthcare services are implemented well and equal access is provided to every person coming to avail the healthcare services.
Palliative care/ end of life care
Within the chronic illness, palliative care is highly needed and is necessary for nurses to implement the same. It is because during chronic kidney disease, there are not many treatments available, and this demotivates the patient. Hence, for this, it is necessary that the nurse plays a crucial role in the management of the diseases. The nurses during the end-of-life care need to be very attentive and supportive as they are assisting a person who may die, and in case effective support is provided to such a person, then it helps support the person. The nurse plays the crucial role of monitoring the condition of the patient by conducting continuous tests (Dupuis et al, 2021). Along with this, another role includes support to the families and also working on effective communication. The communication needs to be present effectively as the person is at the end stage, and in case clear communication is provided, then it will be helpful in managing the work better. Also, another role is to provide emotional support and knowledge to the patient so that they are mentally stable and also ensure that their quality of life increases.
CONCLUSION
Lastly, it is concluded that effective support and services need to be present at the time of working with the patient having a chronic illness. The above analysis highlighted that currently, the chronic illness is increasing, and for this empowerment of the patient needs to be improved. This includes empowering strategies like providing education to patients, equitable access to patients and many others. Further, the study outlined that the community resources, like access to healthcare and other resources, are available for all patients. At last, it was concluded that the nurses play a crucial role in palliative care as they provide effective guidance, communication and many other different types of the support as well.
